We work with you to identify the real-world problems as you experience them, helping you identify the solutions available for those problems and how those solutions can be organised to work most effectively in your life. Solutions need to be workable in the real world to be of any use and to be incorporated long term to improve global health and wellbeing including psychological, physical, relational, and financial.

Assertive Outreach is how we organise and deliver care to you, we outreach, via a specialised team to provide highly coordinated support and treatment.

Multi-disciplinary team MDT:

A multidisciplinary team is developed because no one professional can specialise in all areas of mental and physical health, and a team with differing specialities is brought together and managed by us to help you.

Evidence-Based Practice, requires that decisions about health care are based on the best available, current, valid, and relevant evidence. These decisions should be made by those receiving care, informed by the tacit and explicit knowledge of those providing care. Components of Evidence-Based Practice: Best available evidence, Clinician’s knowledge, and skills, Patient’s wants and needs.

Stepped care is a system of delivering and monitoring your treatments so the most effective yet least resource intensive, treatment is delivered to patients first, only ‘stepping up’ the intensity of care or coordinating specialist services as clinically required. How Can it Help You? We value 2 Main Principles associated with stepped care.

Treatment should always be delivered with the best positive outcomes with the least amount of burden on you, the patient. And scheduled improvement reviews must be in place to enable effective stepping up. What’s Different about Our Approach?

Stepped care models can be found in other healthcare systems. Our care model gives you the security of having all services coordinated for you by one manager effectively organizing and directing your personal care team.

Assertive outreach is about designing services around you, your life and your needs. In practice it means we work hard to help you or your loved one to engage with psychological therapy, psychiatry etc. to receive the help you or they need. Maybe we will deliver services in your home or find ways to help you attend sessions via telehealth.

Particularly useful for young people or for people with conditions that have not responded well to other approaches or are resisting treatment and are not engaged. Or maybe it’s just that you are super busy in a high-pressured job. Assertive outreach works to adapt service delivery to compensate for and overcome these issues.

A multidisciplinary team is a unique solution to the complexities of long-term recovery. For you, being part of a multidisciplinary team, which supports you with the right input, at the right time, can be the difference between being in control of your condition or out of control.

Our company will coordinate, direct, and optimize your MDT to ensure efficiency, communication, regulation, and support for your personalized care plan. Each team is managed for you by us until you feel ready to manage it yourself and it produces a helpful plan (care plan) with you that says who does what and when and why.

Who makes up my MDT? A team will typically be made up of you, people in your immediate circle important in your recovery and the professionals who provide support. Each team that is built is specific to your needs and circumstances. And the makeup of your team will change as your needs do.
